This report from the Vera Institute for Justice guides jail staff to utilize a sentinel event review to better understand suicide outcomes and use the information to improve policy and practice.  Highlighting the work of four jail systems, this report provides recommendations for training, collaboration and communication, and other strategies to transform cultures within facilities to promote safety and well-being of the people in their custody and the corrections staff.
