The Utah Zero Suicide Learning Collaborative has put together a Safe Care Transition for Suicide Prevention toolkit aimed at addressing the lack of support too many individuals experience when undergoing transitions in care. The toolkit provides a best practice menu of strategies for providing continuity of care, a practical guide for creating an office protocol for transitions in care, and additional resources.
