This success story details how the National Alliance on Mental Illness New Hampshire (NAMI NH) and New Hampshire Hospital established a care transitions program for youth inpatients.

The ZEST toolkit is intended to provide guidance and support to community behavioral health centers in Texas to implement the zero suicide framework. The toolkit outlines goals for each core component and provides tools and resources to guide implementation activities.

The Utah Department of Human Services has shared sample caring contact cards that they send to their clients. Each includes a graphic and a caring message. 

The Joint Commission has released a new compendium of Suicide Prevention resources, Zero Suicide among them, to support the implementation of National Patient Safety Goals 15.01.01, revised November 2018. 

The Chickasaw Nation has shared sample caring contact cards that include a message in the Chickasaw language. Youth and adult examples are included.

NowMattersNow.org has created a template for Caring Contacts that includes a printable card redirecting recipients to the resources on their website—made for and by those with lived experience.

Bloomington Meadows Hospital of Indiana sends out handwritten care cards to patients after discharge. The care cards, like the two examples here, are created by receptionists, signed by the staff, and sent in an envelope for privacy.
